Frequently Asked Questions

How do I get a DEA number?
Complete the New Application for RegistrationOpens in a new window form (DEA Form 225). The form can be printed out for completion in hard copy or completed online.
  • In Section 1, the address should be the College of Dentistry and the post office box of the applicant's department.
  • Section 6 should reflect that the University of Florida is a fee exempt institution.
  • The Fee Exempt Certifier is Dr. Boyd Robinson, telephone 352-273-6826.
If completing the hard copy, take the form to Amy Dechow, College’s HR Office, D4-9. A letter from Dr. Boyd Robinson will be included in the packet sent to the U. S. Department of Justice, Drug Enforcement Administration. Renewal Applications for Registration (DEA Form 224) can be completed online in the same manner by clicking on "Applications and On-Line Forms."
How do I get a teaching permit?
To obtain a teaching permit, the following information must be submitted to Amy Dechow, College’s HR Office, D4-9:
  • Copy of a curriculum vitae
  • Copy of a dental diploma or original transcript
  • Current basic life support (CPR) certification
  • Verification of completion of the HIV/AIDS course
If the documents are already maintained in the Human Resources personnel file, duplicate copies may be obtained from the file.
Note: The contact person for CPR certification is Lorie Primosch, Periodontology, at 352-273-8365 or lprimosch@dental.ufl.edu. Information on the HIV/AIDS course may be obtained from Amy Dechow, Human Resources, D4-9, 352-273-5785 or ajd001@ufl.edu.
When the above information is received in the Dean's Office, signed documents from the Dean will be added and the packet will be forwarded to the Department of Professional Regulation in Tallahassee. When the original certificate is returned, a copy will be maintained in the Dean's Office and the original will be forwarded to the faculty member.
How do I get a resident permit?
To obtain a resident/intern permit, submit the following to Susan Loffredo in the Office of Education:
  1. Residency/Intern ApplicationPDF (make sure to answer questions A-F)
  2. Copy of dental diploma or dental school transcripts confirming degree if diploma is not available
  3. Current basic life support (CPR) certification (Note: expiration date must be no sooner than 60 days after issue date of permit)
After the above documents are received, the Office of Education will submit them to the Department of Professional Regulation in Tallahassee. When the original permit is returned, a copy is maintained the the resident's permanent academic record and the original is forwarded to the resident.
Resident permits are renewed annually on June 30th.
How do I obtain an NPI (National Provider Identifier) number?
The following information must be provided to Amy Dechow, College’s HR Office, D4-9, either by e-mail (ajd001@ufl.edu), fax (352-392-3070), or hard copy:
  • First-middle-last name
  • Credentials (e.g., D.D.S.)
  • Any other full names used (e.g., married name)
  • UF ID number
  • Date of birth
  • State of birth or country of birth
  • Gender
  • Social security number
  • Name and mailing address of program (e.g., Periodontology and address)
  • Telephone number of the program
  • Any numbers such as DEA, Medicare, Medicaid, etc.
  • Dental license number and state where issued (or resident permit number)
When the NPI number is received via e-mail, it will be recorded in the Dean's Office and forwarded to the faculty member. Note: The applicant must have a Florida teaching permit or a dental license number.
How do I get my desk, file, bookshelves, etc. moved?
Major moves will be handled by the Physical Plant Division. (There will be a charge for Physical Plant services.) Call 352-392-1121 to request moving services from the Physical Plant Division.
Who do I contact if I want my carpets or floors cleaned?
Call Work Management, Physical Plant Division, at 352-392-1121 to obtain a work order for carpet/floor care. For non-emergency services such as this, it is preferable to call during normal business hours.
Who do I call if I see a spill in the hallway or elevators?
Call Housekeeping in the Health Science Center at 352-392-1121.
Who do I call if a toilet is overflowing in the restroom?
Call Work Management, Physical Plant Division, at 352-392-1121 to obtain a work order and indicate that it is an urgent situation. Someone is available seven days a week to assist you.
Who do I call to report an elevator that is not working?
Call Work Management, Physical Plant Division, at 352-392-1121 to obtain a work order. You must provide the elevator car number when calling in an elevator issue. It is located above the door frame on each elevator. Someone is available seven days a week to assist you.
How do I get an office key?
You may either:
  1. Contact your department administrator to request keys.
  2. Contact Larry Abbott in the Dean's Office at 352-273-5800 or labbott@dental.ufl.edu with the following information:
    • Room number(s)
    • Number of keys being requested
    • Recipient(s) of the key(s)
    • Contact person
    • Phone number
    A Key Request Form will be submitted to the Health Science Center Key Shop.
If I am locked out of my office, what can I do?
Contact your department administrator to obtain a key. If he/she is not available, contact Larry Abbott in the Dean’s Office at 352-273-5800. After hours, the University Police Department can be contacted at 352-392-1111 for assistance. Proper identification will be required to gain entry to your office.
Who do I call to take the campus taxi to my meeting?
Faculty and staff may reserve the campus taxi for official business by calling Work Management, Physical Plant Division, at 352-392-1121, between 7:15 a.m. and 4:15 p.m. Advance notice is required.
How do I get an Official Business Parking Permit for my department?
Jean Sweitzer, Chief of Staff, orders the Official Business Parking Permits for the college in June of each year. The permits run from July 1 through the following June 30. The university now assesses a charge for each permit received by the college. When the permits have been received, each department will be contacted. A representative from the department may pick up the permit in the Dean's Office, D4-4, with signature of receipt.
How do I reserve a conference room?
The easiest way is online. Using the college e-mail system, select "New > Meeting Request." Complete the following fields: "To" (e.g., D4-16), "Location" (same), "Start Time," "End Time," and "Subject." Select "Scheduling Assistant" to see if the room is available. (colored areas signify unavailability.) Click "Send" to forward the request to the appropriate calendar.

How do I make an appointment with Dean Dolan?
All requests are processed by Dr. Dolan's administrative assistant, Tammy Winkler, who can be reached at 352-273-5802 or twinkler@dental.ufl.edu. Faculty requesting appointments are kindly asked to first meet with their chairperson prior to meeting with the dean.
