The Department of Oral & Maxillofacial Surgery is located in the Dental Tower of University of Florida Health Science Center in room D7-6.

You can make an appointment for a consultation/evaluation with a faculty member or resident at 352-273-6731, option 8. Oral surgery is a referral clinic and should receive referrals by fax at 352-392-6909 to the attention of the Receptionist. Appointment date, time and appropriate forms will be sent via mail.
