| Media Reserve Procedure
HSC Library Media Reserve Procedure
Beginning in the Fall 2002 Semester, all course instructors
should have a Media
Reserve Request Form filled out and signed for the items they
would like placed on reserve in the library. If the instructor
has multiple classes in the Fall, a Media
Reserve Request Form must be filled out and signed for each
course.
Beginning in the Spring 2003 Semester, this policy will
become more stringent. If a signed Media
Reserve Request Form is not on file for a course for that
semester the items will be removed from Media Reserve and returned
to the instructor. There will be no exceptions. In addition, all
items from the Fall 2002 Semester will removed and returned to
the course instructor unless they have been renewed for the Spring.
By the Fall 2003 Semester
Each semester, faculty will be responsible for filling out the
Media
Reserve Request Form and bringing any items to Media Reserve
that they wish to place on reserve. This should be done approximately
two weeks prior to the beginning of the semester. Faculty must
give at least ten weeks notice if the library is to purchase material.
It will be the instructor's responsibility to ensure that copies
are in compliance with the "fair use" provisions of
Section 107 of Appendix 1 of the Amended 1976 Copyright Act. By
signing the form, the instructor certifies that he/she is in compliance
with copyright policy (stated on the back of the Media
Reserve Request Form).
At the end of each semester, instructors will be
notified to pick up their materials. After two weeks, if the items
have not been picked up, a second notice will be sent. After one
month, unclaimed reprints will be discarded, and personal books
will be considered abandoned and cataloged into the library's
collection.
If materials are to be used for the subsequent semester, the
instructor will be asked to fill out the Media
Reserve Request Form. If this is not done within one week
of notification, the items will be removed from the Media Reserve
shelves and the instructor will be contacted to pick them up.
