Appointments
& Cancellations
The
office is open Monday through Friday except State Holidays.
The reception area opens at 7:30 AM and closes at 5:30 PM.
Appointment hours are from 8:00 AM to 4:30 PM.
Please call (352) 273-7950 for an appointment.
In order to better
serve you, our office will be closed occasionally so our doctors
and staff may attend continuing education programs and professional
seminars to increase our skills and knowledge of the latest technology,
techniques and practice management.
Cancellations
Our primary goal is to assist
you in attaining and maintaining optimal oral health. Therefore,
your appointment time is reserved exclusively for you. We trust that
no change in your appointment will be necessary and we will call you 48
hours in advance to confirm your reserved time. Should an unforeseen
circumstance cause you to need to change your reserved appointment, we
ask that you give us 48 hours notice.
Hygiene appointments:
If you fail to keep an appointment or give less than 48 hours notice for
a cancellation, there will be a $50.00 charge.
Dental appointments:
If you fail to show for two appointments or give less than 48 hours notice
for a cancellation, you may be dismissed from Faculty Practice.
Changes
in Medical History
At your initial and each subsequent
visit to our office, we will review your medical and dental health
history, perform a blood pressure and oral cancer screening.
To protect your health, please advise us of any changes in your
medical health history, changes in or addition of prescription/other
medication, any surgeries or hospitalizations since your last visit.
Financial
Policy
While providing the very best
quality of dentistry, we strive to keep our fees fair. Our
fees are structured like many private dental practices in our community.
In other words, we are a full-fee practice. When treatment is rendered,
we ask that you pay at the time services are provided. If you have
a dental insurance plan, please ensure that we have the correct and up-to-date
insurance information. While we do not accept insurance as a form
of payment, we will be glad to assist you in filing your claim for reimbursement.
In all cases involving the
placement of a dental prosthesis (crowns, fixed bridges and removable),
one-half of the total fee is expected to be paid at the time the impression
is taken. For your convenience, we accept Visa, Mastercard and Discover
and affordable, flexible payment options through Dental Fee Plan.
Payment for services provided
by specialists may vary from the above policy and should be discussed with
the treating doctor. Some specialists accept Medicaid and/or Medicare.
Please consult with those respective departments.
Dental
Insurance
If you have a dental insurance
plan, please ensure that we have the correct and up-to-date insurance information.
While we do not accept insurance as a form of payment, we will be glad
to assist you in filing your claim for reimbursement. The insurance
payments will go directly to you and you will still be responsible for
payment to Faculty Associates.
However, we do accept
Direct Assignment of the Shands Preferred Dental plan and you
will be responsible only for payment of the co-pay if you are
an insured under this plan.

Options
Faculty Practice has partnered
with Dental Fee Plan to offer
our patients a variety of affordable, flexible payment options.
Several payment plans are available; including interest free, no
initial down payment, extended payment plans up to 60 months, and
prepayments allowed anytime.
