Dear Doctor:

In an effort to provide the most complete and effective evaluation and case-specific management recommendations for your patient, a multidisciplinary approach is utilized. Disciplines represented on our Facial Pain Center tem include: dental/orofacial, clinical psychology, physical and pharmacy.

We appreciate the opportunity to participate in the evaluation of your patient. Completion of the referral form below will expedite scheduling.

Upon receipt of the referral request, the patient will be provided with a pre-scheduling screening form. Once submitted, this information will be reviewed by the Center's health care team in order to customize the evaluation on a case-specific basis.

Effective January 1, 1999, the Facial Pain Center will no longer accept assignment from any insurance company, with the exception of Medicare if it is the primary insurance.
